This is a Notice of the Presidential declaration of a major disaster for Public Assistance Only for the State of ARIZONA (FEMA-1581-DR) , dated 02/17/2005.
Incident: Severe Storms and Flooding.
Incident Period: 12/28/2004 through 01/12/2005.
DATES:
Effective Date: 02/17/2005.
Physical Loan Application Deadline Date: 04/18/2005.

The following areas have been determined to be adversely affected by the disaster:
Primary Counties: COCONINO, GILA, MOHAVE, NAVAJO, YAVAPAI
Tribal Nations: Hopi Tribal Nation, The Portion of the Navajo Tribal Nation Within the State of Arizona
